The old version of the form has been removed from the Government website. Starting March 22, 2019 the Government will no longer accept the old version of the form.

The revised application includes the following significant changes:

  2. Every applicant and co-applicant must pay the $85 biometric (fingerprint) service fee with some exceptions.
  3. Every applicant and co-applicant will receive a biometric appointment notice, regardless of age. The biometric appointments will be scheduled closest to the primary applicant’s address.
